The following must be submitted to the traffic police department:
1. Application for a change of identity.
2. Identity document. For example, a passport. If you are a soldier and are doing service under a contract - a military ID. If you are on the territory of Russia temporarily - a foreign passport.
3. Document on registration of place of residence. For example, a passport with a registration mark, a certificate of registration at the place of residence.
4. Medical certificate (form N 083 / U-89).
5. Old driver's license.
