Together with the insurance policy, you are required to issue an accident notification form in accordance with paragraph 41 of the OSAGO insurance rules. It is filled in by all participants in the accident. This is a very important document for obtaining insurance: if such a notice is not completed, then the insurance company will have the right to refuse to pay you.
Instructions
Step 1
It is best to fill out the notice with a ballpoint pen with such pressure that it would be possible to read the written text on the copy without difficulty. Never fill in the notice with a pencil!
Step 2
If there are witnesses in the accident, then in paragraph 7 of the accident notification, you must write their full name, addresses and phone numbers. It is necessary to indicate if the passengers from your car or the car of the culprit act as witnesses.
It is not necessary to fill in the item on witnesses, not if a controversial situation arises, then this can be very helpful.
Step 3
It is better to fill out the accident notification with the second participant in the accident. It needs two signatures, just like the second copy. But, if the second participant refuses to fill out an accident notice or put his signature, then do it yourself. Do not forget to just describe the brand of his car, number and color.
Step 4
After describing the situation that occurred, on the last line of each column, indicate how many checkboxes you have checked. If there are details that are not provided in the column "circumstances of the accident", or there are disagreements with the second participant, then write it all down in paragraph 15 "Remarks".
Step 5
Next, describe the place of the accident: indicate the names of the streets, the presence of road signs, where the cars were before and after the accident.
Step 6
In the notice, you need to record all visible damage and make sure that the second participant does not add something extra to it. The traffic police officer must also note these damages.
Step 7
When everything is filled in, check again that the details of the participants in the accident were written: full name. drivers, addresses, telephones, car brands, license plates. If the vehicle belongs to a legal entity, then you need to indicate its exact name and address, as well as the surname of the driver who drove this car at the time of the accident.
Do not forget to write down the name of the insurance company, the series and number of the CTP insurance policy.
