If one of the drivers forgets to indicate the name and address of the insurance company or puts a cross in the wrong place, then the insurer will have to search for the missing information on its own. The document certifying the accident must be well read. It is recommended to use only one set of forms for two vehicles involved in the accident. This document has two columns. It doesn't matter which one you fill out.

It is best to use a ballpoint pen when filling. Write with pressure. Please note that all information must be displayed clearly on the copy.
