Instructions
Step 1
Write a statement to the director of the automotive school. The document begins with a request for admission to driver training courses for the category of vehicles you need. Next, write your details: last name, first name, patronymic, date and place of birth, registration address and residence address, if they do not match. Provide information about your place of work, as well as the position held and contact phone numbers.
Step 2
Please indicate your passport details below: series, number, by whom and when it was issued. Write down the individual taxpayer number and number of the medical certificate, as well as the date of its issue. At the end, put the date of writing the document and your signature with a transcript. You will also need to provide a medical certificate in the form No. 083 / y and one photograph of 3x4 cm in size. In case of retraining from another category, attach an already existing driver's license to the package of documents.
Step 3
To change your driver's license in the event of a new category opening, you also need to write an application. In this case, you must provide a driver's examination card, a certificate that you have been trained in a car school, a passport, a 3x4 cm photo, a medical certificate, a driver's license.
Step 4
The application must be written to the State Traffic Inspectorate. Here you also indicate your data: last name, first name, patronymic, date and place of birth, address of residence, passport data. Next, state your request to change your driver's license in connection with the receipt of a new category.
Step 5
Below write the series and number of your current driver's license, when and where you got it. Write a list of attached documents. Date and sign. After you receive your new driving license, you will sign again on this statement to confirm that you received the document.
